Omega-3 evidence is complicated. Some large trials (REDUCE-IT) show significant cardiovascular benefit; others (VITAL) show modest or no effect. The dose, formulation (EPA vs DHA), and baseline dietary intake all matter. Higher-dose EPA-only formulations show the strongest signal. At the population level, higher omega-3 index is consistently associated with lower mortality.
